All Grubscrews / Socket Sets, with the exception of very few custom made items, are measured from end to end, including drive head / end, and any point i.e. the total length of the screw. This measurement gives us the Length. The diameter is the thickness of the screw thread  or it's metric or imperial designation with a size code. Examples of this would be M4 x 10 - this being a Metric 4.0mm thread width by 10mm in length. 3BA x 1/4" would be British Association size 3 thread - by a quarter of an inch in length. 5/16" BSW x 1/2" would be 5/16 (or 0.031) of an inch - British Standard Whitworth thread by a half of an inch in length.
